If you are leasing any part of the uniform in whole or in part, you are REQUIRED to volunteer. Failure to volunteer will result in a forfeiture of your rental fees.

2025–2026 Cheer Uniform Rental Fee Form
This form is for families who are choosing to rent a cheer uniform for the 2025–2026 season rather than purchasing one outright. The rental option allows athletes to use a school-issued uniform for the season, which must be returned at the end of the year.
I acknowledge that I am in receipt of a Cheer Uniform owned by KMSCC Booster Club. I understand that this uniform is on loan for the Cheer season beginning at the first event and ending after the last pep rally and that I do NOT own this uniform but that my $200 is being held for the year for the Lease.
I understand that as a condition of leasing, the Cheerleader and at least one parent/guardian must volunteer and participate in each Fundraising event as the funds used to support the Leasing program and purchase uniforms for Lease come directly from Fundraising and Sponsorship efforts. I understand that if I fail to participate in fundraising, the Leasing Fee of $200 will be retained by Booster in lieu of my participation.
I understand that it is my responsibility to maintain the uniform in performance quality and return it as such - no stains, tears, demarcations, alterations, or other damage and return it clean and freshly washed within 24 hours of my last performance.
I understand that if the uniform is damaged but repairable, I will be charged the cost of repair from my leasing fee and a receipt of that repair will be made available to me.
I understand that if the uniform is damaged and requires replacement, as determined by the Coach, then I will be responsible for the current cost of replacement by Varsity apparel or other vendor for the item to be replaced. I will be provided with an invoice or portal by which to pay, showing the accurate balance for replacement of each item.
Please submit payment by the stated deadline to ensure your cheerleader receives their uniform on time. Thank you for supporting KMS Cheer and helping our athletes represent their team with pride and unity.
